You can refer a client for co-case management or request a secondary consultation. See information below or download our referral pathways information sheet.
How to refer a client
To refer a client to our case management services:
- Phone our intake team on 1800 755 988 to discuss your client’s needs.
- Complete our online client referral form.
- If you are a worker from a family violence service and you have already completed the risk assessment and consent form for your client, please attach the documents to the referral.
- One of our intake workers will then contact the client by phone, using an interpreter if needed.
How to request a secondary consultation
To request a cultural consultation/secondary consult:
- Phone 1800 755 988.
- Complete our online cultural secondary consultation form.
The intake team will record your request and a case manager will contact you within five working days.
